How do you all keep track of your CE hours? Is there a form I can download somewhere? If not what information do I need to write down, and do I need the instructors to sign it or what?

I came on to ask a similar question. Does anyone have or know of any programs (preferably Palm PDA) that tracks CEUs and certs?

Aerial - almost every place is different. The state of VA has an online system we can track how many Con-Ed hours we have credit for (Based on filling out little "Buble Sheets" at each class that are returned to the state). Also they offer a booklet that you can use to track classes.
If you have to track your classes yourself, at a minimum I would keep track of Date, Topic, # of Hours, and instructor name. If there are any other things like course Numbers I would record that as well.
You could also computerize your list using a spreadsheet or wordprocessing program if you'd like.

Never used it - don't know a thing about it but it did turn up on a quick google search for "Palm OS" and con-ed.
You might try running more searches using variations of that.

If I recall correctly on the EMS village website under files or something there is a National Registry excel program that tracks hours.

A lot depends on your state's requirements (not sure about National Registry).
The state of Alaska requires that CMEs be tracked on a form that's included in the recert package. Each class/activity needs to be signed by either the instructor, chief, training officer, etc.
