Default Do CMNs for transport of ESRD patients need to be signed by nephrologist?

The federal reg says the CMN needs to be signed by an "attending physician" but I am not sure whether that means any senior physician (e.g., patient's primary care physician) or the physician referring the patient to dialysis (i.e. the nephrologist). Anyone have an answer for this?

Any doc will do, as far as I know, but I would certainly recommend consulting a billing expert.
